Dental Claims and Benefits Information

United Concordia (UCCI) is a dental benefits vendor, which employer groups may
use to manage their plan participants' dental benefits. UCCI maintains a Web
site for dental providers. "My Patients' Benefits" gives dental providers
instant, secure access to patient information, such as:

Providers must register on the United Concordia site to access the system.
How do I access claims and benefits information for my patients?
UCCI requires you to enter your patient's dental plan ID number to access
benefits and claims information. Because the UCCI system uses a different
member ID number than the number on the patient's
dental plan ID card, you will need to convert the ID number. Enter the
ID number from the patient's dental plan ID card in the text box below. The
number on this card will not work on the UCCI site. You must use the number
returned to you after you hit Submit.

How do I register on the United Concordia site?
UCCI's "My Patients' Benefits" is available to dental care providers and their
office staff only. Dental providers are responsible for maintaining the
security and confidentiality of their user ID and password.
Complete the registration form linked from the "My Patient's Benefits" log-in
page on the UCCI site. To complete the form, you will need a UCCI-assigned
provider identification number. Enter the number portion only. (Do not include
the alpha-prefix.) You also will need a tax ID number or Social Security
number, whichever was submitted on your application to receive a provider
identification number from UCCI.
