Your member ID number is located on your card as shown in the illustration with
highlighting. It has three parts:
- Prefix: This consists of three letters. Some ID numbers do not have prefixes.
Entering the prefix in the first box is optional (green highlighting in image).
- Member ID: This is a nine-digit number. Enter it in the second box (yellow
highlighting in image).
- Member Number: This is a two-digit number that appears at the end of the
member ID number. Enter this number in the third box (blue highlighting in image).
